PRC ID Renewal for Professional Teachers Guide For Claiming

This article is a guide for those who will claim the renewed PRC ID for Professional Teachers in the Philippines. The guide is based on my experience when I was requested by my aunt, who is a Professional Teacher, to claim her new PRC ID. At that time, she has lost her claim slip.

The following is posted on a window (*Window 16) for claiming of the Professional Teachers' PRC ID:

"Please be informed that the printing of renewed license cards be done only upon claiming, after seven (7) working days or as you may come. For your convenience, we advise you to call telephone no. *2557397 or *2535330, 1 day before claiming.

The following documents are needed upon claiming of Teacher's Professional ID:

1. Claim Slip
2. Valid ID of the Owner
Could be one of the following:
   -Expired Professional ID
   -Company ID
   -Driver's License
   -School ID
   -Passport
   -Postal ID
   -SSS/GSIS ID

If a representative is authorized by the owner to claim for his/her Professional ID, these are the requirements/documents needed:
1. Claim Slip
2. ID of the Owner (Teacher)
3. ID of the Authorized Representative
4. Special Power of Attorney

If the claim slip is lost, the owner/authorized representative will need to do the following:

1. Upon arrival at the PRC office, ask assistance from HelpDesk.
2. An "Affidavit of Loss" form will be provided.
3. Fill it up and the following information are also needed:
-Date the owner was registered as a teacher
   -License Number
4. Pay for documentary stamp
5. Claim the ID using this document together with other necessary requirements/documents needed mentioned above.

Note:
Asterisk (*)  - means that the reference is from the PRC Office in Cebu City, Philippines. It may be change and that other PRC offices will have different numbers and may have a different process.
